The following materials may be used during your session: silicone prosthetics (Dragon Skin), adhesives (Pros-Aide, Spirit Gum), alcohol-activated and water-based body paints (Skin Illustrator, Kryolan Aquacolor), sealers, and removers. All products are professional-grade, hypoallergenic, and latex-free.

Timing. The full session takes approximately 2.5–4 hours depending on the package. Please arrive on time. Arrivals more than 15 minutes late may result in a shortened session with no refund.
No phones. Personal phone use and recording are not permitted during makeup application. Movement interferes with precision work. All professional photos and videos are created by the Artist.
Unfinished work. Images of unfinished makeup do not represent the final result and may not be shared publicly.
Cancellation. Cancellations less than 48 hours before the appointment forfeit the deposit. Rescheduling is available with 48+ hours' notice.
Minors. Clients under 18 must be accompanied by a parent or legal guardian who co-signs this form.
I consent to the collection and storage of the personal information provided on this form (name, contact, allergy information) solely for the purpose of providing the SFX makeup service and maintaining session records. My data will not be shared with third parties. I may request deletion of my data at any time by contacting the Artist directly. This is in accordance with the California Consumer Privacy Act (CCPA).
I understand that SFX makeup involves applying silicone prosthetics, adhesives, and body paints to my skin. While all materials are professional-grade and skin-safe, I acknowledge that minor skin redness, temporary staining, or mild irritation may occur, and that individual reactions cannot always be predicted.
I confirm that I have disclosed all known allergies and sensitivities above. I understand that failure to disclose relevant information releases the Artist from responsibility for any resulting reaction.
I voluntarily assume all risks associated with this session and release Ekaterina Antonova from any liability for claims arising from the ordinary risks of SFX makeup application, except in cases of gross negligence or intentional misconduct (per California Civil Code §1668).
